NDIS Incident Reporting: What's Reportable, and When

A clear guide to incident management under the NDIS - the difference between an incident and a reportable incident, the notification timeframes, and how to build a register that protects participants and your registration.

Incident management is a core obligation for registered NDIS providers - and one auditors scrutinise closely. The goal is twofold: keep participants safe, and demonstrate that when something goes wrong, you record it, act on it and learn from it.

Incident vs reportable incident

An incident is any act, omission or event that harmed or could have harmed a participant. A reportable incident is a defined subset that must be notified to the NDIS Quality and Safeguards Commission - including death, serious injury, abuse or neglect, unlawful sexual or physical contact, sexual misconduct, and the use of an unauthorised restrictive practice.

Notification timeframes

Reportable incidents have strict timeframes - generally within 24 hours for the most serious categories, with a fuller report to follow within five business days. Unauthorised restrictive practices have their own notification rules. Because the clock starts when you become aware, the date and time you record matter.

What a good incident register captures

  • What happened, when, where and who was involved.
  • Immediate actions taken to ensure safety.
  • Whether it's reportable, and the date the Commission was notified.
  • The investigation, outcome and any corrective actions.
  • A status lifecycle - open, in review, closed - so nothing stalls.

Turn incidents into improvement

Recording an incident is the floor, not the ceiling. The providers who impress auditors can show a thread from incident to corrective action to a changed practice or policy. That continuous-improvement loop is the point of the whole system.
